A1 first layer print issues on multiple plates

I noticed a growing problem in my lightboxes to the point where i finally decided it was bad enough to diagnose and resolve.

I made sure bed was cleaned with soap and hot water, didnt help. I calibrated four different filaments, which helped a lot. Then siddenly, the issue reappeared. I found a video that shows removing the hot end, remove the three screws behind it, and then tighten the four screws (which were noticably loose) - this eliminated the random issues, the calibrations also helped, and I realize now how oil from fingers can create certain symptoms. However, I noticed there is a spot that is under extruding. I thought maybe the print plate had a dent, so i tried other plates, same issue in the same spot.

I am using a scaled 240x240x.2mm primitive cube in bambu slicer, using a few different PLA filament brands (Inlamd, Elegoo, Anycubic) that have all been calibrated in the stock .4mm nozzle. I tried smooth plates and textured plates. I even tried cleaning the bed under the print plates. I thought about tramming, but this issue is so small and isolated, im not sure tramming would help and that by adjusting the bed, I might create more problems.

What do you all think? Is the bed potentially dented?

It could be a cold spot, but that’s just a guess.
