Welcome back @ExtremeElementz, it’s been a while. 
There has been numerous reports on bearings sliding out of place. Most of the reports indicate a bed leveling or build plate missing error.
There was a long post I made about it here.
While I am not saying that is your problem, it may be the inverse of the problem I described in that post. You’ll want to check the bearings are flush. Here’s what I mean in your photos. Your left bearing is perfect. It would be important to verify the two others. They don’t have to be perfect but they have to be even with each other.
